Role Overview We're hiring a full-time PMHNP-BC to provide on-site behavioral health services for residents in Assisted Living/RCFE communities across LA and OC, with consistent, predictable volume.
What You'll Do- Psych evals and follow-ups; medication initiation, titration, and monitoring
- Coordinate with PCPs, facilities, families/POAs, and our concierge team
- Same-day documentation in EMR; regulatory/CMS compliance
- Staff education (behavior plans, safety, de-escalation basics)
Requirements- PMHNP-BC; active CA NP and RN licenses; active furnishing number, NPI, DEA, BLS
- Senior living/ALF/RCFE experience preferred; EMR proficiency
Compensation - Annual base salary: $160k - $175k (based on experience)
- Monthly performance bonus
- W-2 with full benefits; mileage reimbursed per policy
- Final terms outlined in the employment agreement
Schedule & Setting Full-time, Monday through Friday. Clustered LA and OC routes, with occasional telehealth visits.
