The Provider Relations Manager - CKD will serve as a field-based, market-facing relationship leader responsible for developing, managing, and deepening strategic partnerships with nephrology practices, primary care groups, dialysis providers, hospitals, and other CKD-related referral partners across the Ohio market.

This role is ideal for a candidate with strong provider-facing experience, including pharmaceutical, biotech, medical device, specialty pharmacy, managed care, or healthcare account management experience. The successful candidate will bring a disciplined territory-management approach, strong clinical curiosity, and the ability to engage providers in meaningful conversations around CKD progression, cardio-renal-metabolic risk, care coordination, and value-based care.

The role will focus on identifying and prioritizing high-value nephrology and CKD referral partners, educating practices on AbsoluteCare's CKD care model, supporting provider onboarding and engagement, and using data to drive relationship strategy, practice-level follow-up, and measurable provider engagement outcomes.

Through AbsoluteCare's Beyond Medicine™ model, this individual will help advance partnerships that support earlier identification, coordinated care, member engagement, and improved outcomes for patients living with chronic kidney disease.

Duties and Responsibilities

Provider Engagement, Education & Account Management
  • Build and maintain trusted relationships with nephrologists, advanced practice providers, practice administrators, office managers, dialysis partners, primary care providers, and other CKD-related specialists across the assigned market.
  • Conduct proactive field-based outreach to priority nephrology practices using a structured territory plan, account segmentation strategy, call plan, and follow-up cadence.
  • Educate providers and practice teams on AbsoluteCare's CKD program, care model, member engagement approach, referral pathways, and value-based partnership opportunities.
  • Engage providers in clinically informed conversations related to CKD progression, cardio-renal-metabolic risk, medication adherence barriers, care gaps, nephrology access, dialysis transition planning, and avoidable utilization.
  • Leverage familiarity with therapeutic areas commonly addressed in CKD and cardio-renal-metabolic care, including SGLT2 inhibitors, RAAS inhibition, mineralocorticoid receptor antagonists, GLP-1 receptor agonists, diabetes, hypertension, heart failure, and renal risk management.
  • Translate clinical and operational program information into clear, provider-relevant messaging tailored to nephrology practices, primary care offices, and referral partners.
  • Host provider meetings, lunch-and-learns, practice touchpoints, joint operating reviews, and market-facing education sessions to promote clinical alignment and operational partnership.
  • Serve as the primary relationship contact for assigned practices, ensuring timely follow-up, issue resolution, and ongoing engagement.
Nephrology Practice Targeting & Network Development
  • Identify, prioritize, and engage nephrology practices and CKD-related provider groups that can support AbsoluteCare's CKD program growth and member outcomes in Ohio.
  • Develop and maintain a market-level provider targeting plan, including priority nephrology accounts, referral sources, practice contacts, relationship status, opportunity stage, next steps, and barriers to engagement.
  • Apply territory-management and account-planning discipline similar to pharmaceutical or specialty sales models, including pre-call planning, account research, stakeholder mapping, objection handling, and post-call documentation.
  • Partner with the Market President, Medical Director, clinical leadership, analytics, and operations teams to evaluate partnership opportunities and align high-performing providers to AbsoluteCare's CKD workflows.
  • Support the development of strategic relationships that improve specialty access, care coordination, referral connectivity, and member-level follow-through.
  • Monitor provider satisfaction, relationship health, access issues, referral workflow barriers, and operational concerns; escalate and resolve issues in partnership with internal teams.
  • Ensure appropriate geographic and specialty coverage for the CKD population, with particular focus on nephrology access and practice-level engagement.

Performance Management, Data & CRM Discipline
  • Track provider engagement activity, outreach volume, practice contacts, meeting completion, referral activity, relationship status, and follow-up actions using CRM or designated operational tools.
  • Use internal dashboards, claims/utilization insights, provider performance data, referral patterns, and member-level information to identify opportunities for provider engagement and practice-level improvement.
  • Convert data insights into targeted account strategies, provider talking points, and actionable next steps for internal and external stakeholders.
  • Monitor progress against provider relations KPIs, including provider touchpoints, account penetration, meeting completion, referral pathway development, issue resolution, and provider satisfaction.
  • Prepare concise provider account updates, market relationship summaries, and escalation reports for leadership review.
  • Collaborate with clinical, analytics, growth, and market teams to support providers in meeting shared goals related to CKD progression, care coordination, utilization reduction, member engagement, and outcomes.
  • Support joint operating reviews with strategic provider partners, ensuring clear agendas, performance insights, open issues, decisions, and next steps.


Operational Support & Internal Coordination
  • Partners with clinical, operations, analytics, marketing, and member engagement teams to ensure provider-facing materials, referral workflows, talking points, and escalation processes are clear and effective.
  • Maintain accurate provider contact information, account notes, meeting summaries, relationship status, next steps, and issue logs in the CRM or designated tracking platform.
  • Support development of provider-facing materials that clearly explain AbsoluteCare's CKD program, member eligibility, referral process, care coordination model, and value proposition.
  • Help identify and resolve barriers affecting provider collaboration, including communication gaps, unclear workflows, member-level action barriers, or referral process breakdowns.
  • Serve as a connector between external provider partners and internal AbsoluteCare teams to ensure visibility, accountability, and timely follow-through.


Minimum Qualifications

Required
  • Bachelor's degree in healthcare administration, business, life sciences, nursing, public health, or related field; equivalent relevant experience may be considered.
  • 5+ years of experience in one or more of the following: pharmaceutical sales, biotech sales, medical device sales, specialty pharmacy, provider relations, healthcare account management, provider network management, managed care, or value-based provider engagement.
  • Demonstrated success building and managing provider relationships in a field-based, territory-based, or account-management role.
  • Experience calling on or partnering with physicians, advanced practice providers, practice administrators, office managers, or clinical decision-makers.
  • Ability to understand and communicate clinical and operational information in a clear, credible, and provider-relevant manner.
  • Strong account planning, territory management, relationship development, follow-up, and documentation discipline.
  • Ability to analyze data, identify account-level opportunities, and translate insights into actionable provider engagement strategies.
  • Proven ability to build rapport, manage objections, resolve issues diplomatically, and sustain long-term strategic partnerships.
  • High level of initiative, organization, and follow-through; capable of managing multiple priorities in a fast-paced, field-based environment.
  • Proficient with CRM systems, Microsoft Office Suite, Excel, PowerPoint, Outlook, and virtual collaboration tools.
  • Willingness to travel regularly (50%) within the assigned Ohio market


Preferred
  • Pharmaceutical, biotech, medical device, or specialty sales experience in nephrology, endocrinology, cardiology, primary care, diabetes, hypertension, heart failure, CKD, or cardio-renal-metabolic therapeutic areas.
  • Experience promoting or supporting education around therapies relevant to CKD and cardio-renal-metabolic care, such as SGLT2 inhibitors, RAAS inhibition, GLP-1 receptor agonists, mineralocorticoid receptor antagonists, diabetes medications, cardiovascular therapies, or renal-protective treatment strategies.
  • Existing relationships with nephrology practices, primary care groups, dialysis organizations, hospitals, or CKD-related provider partners in Ohio.
  • Understanding CKD, ESRD, dialysis care coordination, specialty referral patterns, transitions of care, medication adherence barriers, and social drivers impacting complex populations.
  • Familiarity with value-based care, managed Medicaid, Medicare Advantage, risk-based programs, quality metrics, utilization reduction, and care coordination models.
  • Experience using Salesforce, Tableau, Power BI, or other CRM/reporting platforms to manage account strategy, performance, and follow-up.
  • Strong presentation skills with ability to conduct provider meetings, practice education sessions, and executive-level account reviews.
