Job Title: Primary Care Physician (MD/DO)
Location: Rochester, NY (West Ridge Rd), clinic-based, in-person
Reports To: Senior Medical Director
The Opportunity
This is not a traditional Primary Care Physician role.
Town Square Health is hiring a Primary Care Physician (MD/DO) for our new West Ridge Road location in Rochester, NY. You'll deliver high-quality primary care in a Medicare-focused, value-based care environment, with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20 to 40 minute visit lengths based on patient needs, and a team-based care model with two Navigators paired to each provider.
Our Navigators are an expanded Medical Assistant role. Alongside the clinical support you'd expect from an MA, each Navigator serves as a dedicated partner to their patients, building long-term relationships and supporting chronic disease management between visits.
You'll join at an early stage with a chance to help build a care model from the ground up. That means real ownership, real influence, and the opportunity to shape how clinical excellence comes to life at TSH.
You'll have the opportunity to:
- Deliver excellent clinical care with the time and team support to practice comprehensive medicine
- Coordinate across the care team, partnering closely with your Navigators and specialty colleagues
- Help shape a new care model designed to improve outcomes and patient experience
What You'll Do
As a Primary Care Physician at TSH, you'll own the care of your patient panel in a clinic built around relationships, not volume. You'll lead a team-based care model, use AI-enabled tools like AI scribes, and help build the clinical workflows our future clinics will follow.
Key responsibilities:
Patient care
- Deliver comprehensive, high-quality primary care to Medicare-eligible patients, with visit lengths tailored to each patient's needs
- Manage chronic conditions and preventive care with a focus on long-term outcomes
Team-based care
- Partner with your two Navigators to support patients between visits using AI-enabled workflows
- Coordinate with specialty colleagues during simultaneous visits and across the care team
Building the care model
- Help shape the clinical workflows and culture of a new clinic
- Share feedback that improves outcomes and patient experience as TSH grows
Who You Are
You are a primary care physician who wants the time to practice medicine the way it should be practiced. You value long-term relationships with your patients, thrive in a team-based environment, and are excited to help build something new.
Must-have:
- M.D. or D.O. degree
- Board certified in Internal Medicine, Family Medicine, or Geriatrics
- Active New York State medical licensure in good standing, or must obtain prior to start date
- Applied for DEA registration prior to hire/start date
- Must meet all applicable employer and payer credentialing requirements
- Ability to meet post-offer requirements, including drug screen and health assessment, and compliance with all New York State health requirements
Nice-to-have:
- Experience in value-based care or Medicare Advantage
