CenterWell Senior Primary Care, a subsidiary of Humana Inc., is looking for a Primary Care Nurse Practitioner or Physician Assistant. You will work onsite in one of the 8 primary care centers in The Villages, FL and report to the Physician Lead. You will enjoy a team-based practice model, manageable patient panels, and a stable Monday-Friday 8:00am-5:00pm outpatient schedule with no nights, weekends, or hospital call.
Responsibilities
- You will deliver comprehensive outpatient care to adult and senior patients.
- You will maintain accurate medical records and documentation.
- You will diagnose and manage moderately to complex medical conditions.
- You will work with interdisciplinary teams to ensure holistic care.

Required Qualifications:For nurse practitioner:
- Active, unrestricted Nurse Practitioner license in FL, or ability to obtain
- Current national certification (AANP or ANCC)
For Physician Assistant:
- Active, unrestricted Physician Assistant license in FL, or ability to obtain
- Successful completion of physician assistant practitioner training and certification as a practitioner (general or specialized) from an approved physician assistant program.
For both:
- DEA license (or ability to obtain)
- This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Preferred Qualifications:- Experience in primary care, family medicine, internal medicine, or geriatrics
- Comfort working with Medicare and managed care populations

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$123,800 - $170,300 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
