Full Job Description
Under the direction of the Senior Population Health Analyst, the Population Health Analyst II leverages data-driven insights to enhance patient care, support Quality and Performance Improvement initiatives, and advance population health strategies in Ambulatory Care. This role involves collaborating with stakeholders to analyze data, generate reports, and identify care gaps while also assisting in the configuration, optimization, and support of Epic Population Health applications to ensure seamless integration and effective system utilization.
  • Collaborate with leadership, practice management, IT, and other stakeholders to collect and analyze data to improve the quality of clinical care.
  • Support the collection, transfer, analysis of data with internal and external IT stakeholders.
  • Manage daily, weekly, and monthly data-management tasks, including running dashboards, reports, and data analysis using extracts from various sources.
  • Review electronic medical records to confirm if patients are missing services, and develop reports for scheduling, or a supplemental data report where care has already been provided.
  • Conduct analyses as requested by senior leadership, supporting special quality improvement projects.
  • Develop presentations, diagrams, and flowcharts for clinical staff to follow, ensuring clarity in process improvements.
  • Support the Epic electronic medical record development and implementation, ensuring optimal functionality for Population Health initiatives.
  • Collaborate with stakeholders to analyze business and clinical needs, interpreting end-user requirements to facilitate Epic-related solutions.
  • Builds Epic records, features and functions under the direction of the Senior Population Health Analyst.
  • Configure application settings within Epic to align with clinical and operational needs, ensuring seamless integration with Population Health workflows.
  • Implements comprehensive test plans, execute tests for new systems, version upgrades, and modifications, and document all testing outcomes to maintain system integrity.
  • Provide support during the implementation of new Epic applications, programs, and system updates, assisting with troubleshooting and system optimization.
  • Interface with users across the SBH Health System, addressing inquiries, providing Epic-related support, and conducting training sessions on new applications, modules, and functionalities.
  • Provide on-site support within SBH Health System and affiliates as required, offering immediate assistance and troubleshooting for both Epic and Population Health-related concerns.


  • Education:
    • Bachelor's degree, or
    • An Associate's degree with 2 years' data management and analysis experience.
  • Epic certification in Healthy Planet, Compass Rose, Cadence or related app (maintaining all requirements for Epic certification is a condition of employment).
  • Experience in healthcare quality settings with data or statistical analysis, data manipulation, and reporting is preferred.
  • Candidates must have excellent advanced skills with Microsoft Excel, and be proficient at vlookup, pivot tables and formulas.
  • Critical thinking, problem solving and analytical skills.
  • Strong interpersonal skills.
  • Excellent written and verbal communication skills.
  • Excellent project management skills.
  • Knowledge of managed care, population health, and value-based care preferred.
