  • Develop and implement goal-oriented, evidence-based treatment plans in collaboration with clients and caregivers
  • Supervise and monitor physiotherapy assistants when appropriate
  • Educate clients and caregivers, connecting them to community resources
  • Maintain accurate documentation of assessment findings and care plans
  • Ensure documentation aligns with referral source requirements and CPO standards

Job Description
  • Provide ministry-funded, community-based physiotherapy services within clients' homes and/or through virtual service delivery
  • Complete comprehensive physiotherapy assessments using interviews, standardized tools, and informal assessment techniques in areas including:
    • Geriatrics
    • Neurology
    • Orthopedics
    • Palliative care
  • Develop and implement goal-oriented, evidence-based treatment plans in collaboration with clients and caregivers
  • Assign, supervise, and monitor physiotherapy assistants, when appropriate
  • Provide education to clients and caregivers and connect them to appropriate community resources
  • Maintain accurate, timely, and professional documentation, including:
    • Assessment findings
    • Care plans
    • Progress and discharge summaries
    • Documentation aligned with referral source requirements and College of Physiotherapists of Ontario (CPO) standards
