Summary

Atlantic Medial Management (AMM) is looking for a Physician Assistant or Family Practice Nurse Practitioner for our Family Practice clinic in Cary, NC. AMM is a corporate medical management firm that is dedicated to improving patient care. This is a great place to have that perfect balance between work and personal time. Our office is open Monday - Friday from 8 am to 5 pm, our salary range is competitive, and we take experience into consideration. Under general direction, provides quality patient care; conducts physical examinations, completes health history summary, documents patients' progress, and assists with patient care management.

Duties & Responsibilities

  • Manages patient health care within protocols mutually agreed upon by physician and physician extenders; assesses health status of patient utilizing advance physical examination and medical history skills and designs individualized health care management plans in collaboration with other health professionals; promotes the role of the patient and practice within a PCMH including setting goals and assisting patenting in understanding the tailored, individual care plan.
  • Provides patient with written clinical summaries at relevant office visits, such important or chronic condition; well-childcare visits for pediatric patients; and visit that result in a change in treatment plan or goals.
  • Responsible for assessing and as appropriate, refers patients to other resources (external or internal) for care management support, such as disease management (DM).
  • Using motivational interviewing techniques to assess patient readiness to change and self-management abilities, to include the use of questionnaires and self-assessment forms and tools.
  • Responsible for systematically monitoring and providing follow-up on tests, labs, etc. that requires action and are performed. Assists with patient care management; provides follow-up care to patients; updates physicians on patients' status; Assists clinical staff in pre-care and post visit planning activities.
  • Keeps informed of patients' progress and documents progress in patients' EHR; interacts with other health care agencies and organizations to assist in providing the highest quality of care to the patient.
  • Required to follow standing orders for services outlined by practice guidelines and procedures such as: testing protocols, defined triggers for prescription orders, medication refills, vaccinations, and routine preventive services.
  • Remain actively involved in contributing and supporting QI activities to contribute to the enhancement and continuity of care.


Minimum Qualifications

  • Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program.
  • One year in family practice preferred.


Licensure/Certification Requirements:

  • Licensed as a Physician Assistant in the state of North Carolina or current unrestricted North Carolina RN license and NP license.
