Are you a dedicated Physical Therapist Assistant (PTA) looking to use your skills to improve patients' mobility and quality of life? Join our compassionate home health team, where you'll work closely with a supportive, interdisciplinary team to help individuals regain strength, reduce pain, and restore function-all in the comfort of their own homes.
Your Role & Responsibilities:- Implement individualized physical therapy care plans under the supervision of a licensed PT.
- Guide patients through prescribed exercise programs to improve strength, range of motion, and mobility.
- Assist patients in gait training using walkers, crutches, braces, and other mobility aids.
- Apply therapeutic modalities such as ultrasound, muscle stimulation, hot/cold packs, and massage.
- Monitor and document patient progress, reporting significant changes to the PT.
- Educate patients and caregivers on home exercise programs to encourage continued progress.
- Maintain sterile procedures and ensure therapy equipment is properly cleaned and maintained.
- Participate in quality assurance (QA) and in-service training to stay up to date with best practices.
Qualifications:- Licensed Physical Therapist Assistant (PTA) in the state of practice.
- Graduate of an accredited PTA program (CAPTE-approved).
- Minimum 2 years of experience as a PTA (home health experience preferred).
- CPR certification (required).
- Ability to lift and transfer patients weighing 150+ lbs.
- Strong communication and interpersonal skills to work effectively with patients and families.
- Must pass a criminal background check and meet all state compliance requirements.
- Comfortable traveling to patient homes (reliable transportation required).
