You'll help residents rebuild mobility after a hospital stay and address ongoing movement challenges, with treatment grounded in their daily needs.
What You'll Do- Evaluate functional mobility and identify each resident's treatment priorities
- Develop individualized plans for strength, balance, walking, and transfers
- Deliver therapy and adjust interventions based on progress and tolerance
- Guide residents and caregivers on safe movement and appropriate assistive devices
- Maintain accurate clinical records and coordinate with nursing and the rehab team
