Full Job Description
We are seeking a dedicated and compassionate Physical Therapist to join our team in a skilled nursing facility.

RESPONSIBILITIES:
  • Conduct comprehensive evaluations of patients to identify physical, functional, and rehabilitation needs.
  • Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
  • Monitored and documented patient progress and adjusted treatment plans as necessary.
  • Educate patients and caregivers on exercises and techniques to promote recovery and prevent future injury.
  • Provide hands-on therapy, including exercises, manual therapy, and modalities as appropriate.
  • Ensure compliance with regulatory standards and company policies in all aspects of care delivery.
  • Participate in clinical meetings and contribute to a culture of continuous improvement.


QUALIFICATIONS:

  • New passer/graduates are welcome
  • With Skilled Nursing or Long-Term Care, experience is a plus
  • Previous experience as a Physical Therapist (PT) is a plus
  • Physical Therapy Degree from an accredited program
  • State licensure or eligibility for licensure as a physical therapist
  • Is in good standing with all regulatory agencies and licensing boards as required
  • Interest in working with the geriatric population
  • Ability to work effectively both independently and within a team
