We9re looking for a full-time Physical Therapist to serve patients in Marin County. This individual would be based out of their home while traveling to and from patient homes for clinical care.
Key Responsibilities
- Evaluate patients' functional status, muscle strength, mobility, gait, and ROM to develop and direct individualized treatment plans
- Instruct patients, families, and caregivers on therapeutic appliances, PT goals, and relevant care phases
- Monitor and report patient progress and condition changes to the physician
- Prepare and submit clinical progress summaries and participate in discharge planning
- Identify needs for additional home health services and refer accordingly
- Supervise Physical Therapy Assistants and, when PT is the sole skilled service, evaluate home health aide care every two weeks
- Provide consultation, staff in-services, and participate in peer review as needed
Qualifications
- Bachelor's or Master's degree in Physical Therapy from an accredited program
- Active state PT license
- Minimum 2 years of PT experience (home health/community experience preferred)
- Strong verbal and written communication skills
- Current CPR certification
- Valid driver's license, insured vehicle in good working order
