Responsibilities

  • Compound and prepare pharmaceutical formulations as ordered
  • Consult with physicians about medication order discrepancies
  • Prepare and monitor TPN solutions for patients
  • Mix irrigating solutions and IV additives using aseptic techniques
  • Demonstrate competency in aseptic techniques and hazardous drug preparations
  • Advise medical staff on drug availability and side effects
  • Collect data for Quality Assurance and Performance Improvement
  • Interpret prescriptions and communicate with providers or insurers
  • Provide medication counseling to patients as needed
  • Supervise pharmacy technicians and review their work

Full Job Description
Job Summary:

Performance of professional functions generally associated with the practice of pharmacy in the hosptial setting.

Duties/Responsibilities:
• Compounding and preparing ordered pharmaceutical preparations
• Contacting and conferring physicians if there are any questions or irregularities regarding medication orders
• Preparing TPN solutions and Monitoring TPN Patients as required
• Mixing of irrigating solutions and intravenous additive solutions as required using aseptic technique with the laminar air flow hood or CAL, or CACI for hazardous medications
• Shows competencies in aseptic technique, garbing and gowning, and didactic training in IV and HD preparation
• Consulting with physician and nursing staff concerning availability, form, side effects and incompatibilities of currently available drugs
• Responsible for the collection of Quality Assurance/Performance Improvement data
• Interprets prescription orders and contacts provider or insurer as indicated
• Performs medication counseling for patients as requested
• Supervises activities and checks work of pharmacy technicians
• Performs other duties as assigned

• Minimum of one year experience in hospital Pharmacy preferred
