Full Job Description
Infucare RX is seeking an experienced Pharmacist to join our team in Aston area. As a Pharmacist, you'll play an essential role in ensuring safe, effective, and compassionate care for patients receiving home infusion therapy. You'll collaborate with physicians, nurses, and other healthcare professionals while managing all aspects of the pharmacy process - from verifying prescriptions to monitoring patient outcomes.

This position requires attention to detail, clinical expertise, and a commitment to improving patient experiences through high-quality pharmacy services.

Job Responsibilities:

  • Ensure all pharmacy activities comply with federal and state regulations, accreditation standards, and company policies.
  • Coordinate and facilitate new patient referrals and admissions in collaboration with nursing, customer service, and distribution teams.
  • Review, verify, and process new patient orders for accuracy and completeness.
  • Provide consultation to physicians, nurses, and patients on safe and effective medication therapy.
  • Assess and monitor patient therapy outcomes and communicate findings to healthcare teams.
  • Prepare and dispense sterile and non-sterile medications in accordance with USP standards and professional best practices.
  • Maintain accurate patient records, pharmacy documentation, and quality assurance logs.
  • Participate in quality improvement initiatives, training programs, and staff development activities.
  • Maintain clinical skills through continuing education and professional development.
  • Participate in the pharmacist on-call rotation as needed.
  • Perform other duties as assigned to support pharmacy operations and patient care.

  • Adhere to HIPAA, safety, and quality regulations.
  • Report and document any medication-related incidents or equipment issues promptly and accurately.

Required Qualifications
