Nurse Practitioner/Physician Assistant
Compensation: $122,000 - $162,000
The Patient Engagement Center (PEC) is a centralized contact center supporting appointment scheduling, telephone triage, referral management, and care navigation for Saratoga Hospital Medical Group. The embedded Nurse Practitioner or Physician Assistant serves as a patient care provider and clinical resource within the PEC, augmenting and extending the services of the health care team. The NP/PA works closely with PEC team members, practice providers, and operational and clinical leaders, with an appropriate level of autonomy in patient care and medical decision-making.
This position will be fully onsite during the training and orientation period, and will have the opportunity to transition to hybrid onsite and remote work thereafter. Remote work schedules are subject to departmental and operational needs.
A few highlights of responsibilities:
Clinical guidance and escalation support: Provides ongoing medical guidance and support to Telephone Triage RNs and serves as an available clinical resource throughout the day for triage questions, clinical decision-making, care navigation, and escalation management.
Telehealth care: Provides telehealth visits when clinic capacity limits in-office access, offering timely evaluation, treatment, and an alternative to a higher level of care when clinically appropriate. Obtains and documents relevant medical history, assesses health risks and needs, performs an appropriate virtual clinical assessment, formulates a differential diagnosis and plan of care, orders and reviews diagnostic tests, prescribes and manages pharmacologic therapies within scope, makes referrals, and reassesses or modifies the plan of care based on patient outcomes.
Quality assurance and case review: Reviews quality assurance reports and conducts triage case reviews. Applies clinical expertise, evidence-based protocols, and knowledge of PEC and practice workflows to identify learning and development opportunities for PEC staff, providers, and practice staff, and to recommend improvements to processes, workflows, protocols, and standards of care.
Clinical workflow development: Partners with the PEC Director, Program Manager, Clinical Coordinator, Primary Care Lead APP, and Medical Director to develop, implement, evaluate, and optimize clinical workflows, protocols, decision-support tools, and operational processes.
Provider and practice engagement: Participates in PEC/provider engagement and advisory forums and attends Primary Care provider meetings at routine intervals to solicit feedback, strengthen relationships, and support alignment between the PEC and provider offices.
Virtual clinical support: Partners with the Virtual Support team to provide in-basket coverage, pre-visit planning support, and other clinical support functions as capacity allows.
Primary Care call coverage: Participates in the call schedules of the Primary Care service line, including three-four weeks of call annually for each of the three Primary Care call groups.
Interdisciplinary collaboration: Participates in team meetings, conferences, departmental committees, multidisciplinary committees, and quality improvement initiatives that influence clinical practice, patient access, and advanced practice provider standards.
Education and professional practice: Serves as a clinical resource to staff, supports educational programming as needed, and maintains current knowledge relative to professional practice, evidence-based care, and applicable organizational requirements.
Licensure and documentation: Maintains required professional documentation and credentials, including applicable licensure, certifications, infection control requirements, ACLS/BLS, DEA registration, and other health system requirements.
