Qualifications

• Master's degree in Nursing and current NP certification from an accredited program.
• Active, unrestricted NP license in relevant state(s) with prescriptive authority.
• 2-3 years of experience in PM&R, geriatrics, or post-acute care; nursing facility experience preferred.
• Strong clinical skills in managing recovery barriers including rehabilitation and chronic conditions.
• Effective communication and time-management for efficient patient encounters.
• Proficiency with EHR systems for remote documentation.
• Ability to work independently with a high level of autonomy.

Responsibilities

• Round in assigned facilities on a consistent weekday daytime schedule.
• Oversee daily patient interactions through timely micro visits for assessments and management.
• Collaborate with physicians and facility staff on individualized recovery plans.
• Complete documentation using Puzzle's EHR, often remotely from home.
• Monitor readmission risk and address gaps in post-acute care compliance.
• Maintain compliance with post-acute care regulations.
