Join a compassionate and highly respected team dedicated to improving quality of life for patients and their families. We are seeking an experienced Nurse Practitioner with exceptional communication and relationship-building skills to provide independent palliative care services, focusing on symptom management, goals-of-care discussions, and patient-centered support in the home setting. This role serves patients throughout Delaware and Montgomery Counties, allowing you to make a meaningful impact where patients are most comfortable. Previous home care, hospice, or palliative care experience is preferred.
Position: Nurse Practitioner, Palliative Home Care Shift: Full Time, 40 hours/week, Day Shift. No Holidays. On call rotation weekends.
Experience: 2 years Nurse Practitioner experience preferred in palliative home care. Minimum of 2 years Nursing experience in palliative home care preferred.
Education: Completion of a Nurse Practitioner program required. MSN Required.
Licensures/Certifications: Licensed by the Pennsylvania State Board of Nursing Examiners as a Certified Registered Nurse Practitioner. Provider BLS (American Heart Assoc/American Red Cross) required. Require ACLS certification. May require PALS (pediatric advanced life support). Successful completion of the MLH credentialing process through the Medical Staff office, with temporary or permanent privileges approved.
