Board Certified as a Nurse Practitioner or Physician Assistant
Experience in palliative care preferred
Ability to work collaboratively in a team environment
Strong communication skills
Proficiency with EHR systems, specifically Epic
Responsibilities
Provide comprehensive palliative care to patients
Collaborate with physicians and other healthcare professionals
Manage patient assessments and care plans
Educate patients and families about palliative care options
Document patient interactions and care in the EHR
