Nurse Practitioner NP Hospice and Palliative Care

Qualifications

• Current, active, and unrestricted RN and NP licensure in the state of practice
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care

Responsibilities

• Collaborate proactively as a team member in a hospice care environment
• Conduct comprehensive assessments of adult and geriatric hospice patients
• Evaluate and manage the plan of care for patients
• Demonstrate a commitment to caring for patients and their families
• Complete documentation and records accurately and on time
• Adhere to established policies and procedures related to scope of practice
• Ensure continuity of patient care in a hospice setting
