Location: Phoenix, AZ
Description: Impresiv Health is seeking a Nurse Practitioner to join an interdisciplinary care team providing comprehensive medical care and chronic disease management to a focused panel of patients living with chronic kidney disease (CKD) and other complex conditions.
This role focuses on preventive care, chronic disease management, patient education, and care coordination through a value-based, relationship-centered approach. The Nurse Practitioner will collaborate closely with physicians, Social Workers, Care Navigators, specialists, and community resources to ensure care is coordinated, connected, and centered around the whole person.
Patient care is delivered through a combination of in-home visits, telehealth, and phone-based support, allowing for meaningful patient relationships and a deeper understanding of each patient's individual health needs.
Schedule and Work Model:
Monday through Friday, 8:00 AM to 5:00 PM, with flexibility based on patient needs
No routine weekends or overnight shifts
Limited after-hours calls
Approximately 5 to 6 patient visits per day
Combination of in-home visits, telehealth, and care coordination
Daily collaboration with patients and interdisciplinary care team members
What You Will Do:
Manage a focused panel of patients with CKD and other complex chronic conditions alongside an interdisciplinary care team
Conduct comprehensive health assessments, medical histories, physical examinations, and ongoing clinical evaluations
Diagnose, treat, and manage acute and chronic conditions within scope of practice
Order, interpret, and review laboratory and diagnostic testing
Prescribe and manage medications in accordance with state regulations
Develop individualized care plans that address patients' medical, behavioral, and social needs
Educate patients and caregivers regarding disease prevention, medication adherence, lifestyle changes, and treatment plans
Coordinate care across primary care providers, specialists, hospitals, skilled nursing facilities, home health, hospice, and community organizations
Participate in regular interdisciplinary care team huddles
Monitor patient progress and adjust care plans to improve outcomes and reduce avoidable hospitalizations
Document clinical encounters accurately and in a timely manner
Support quality improvement initiatives and evidence-based practice
Maintain patient confidentiality and comply with company policies and regulatory requirements
Provide after-hours telephonic support as needed
