We are looking for a Nurse Practitioner to join our growing team of dedicated professionals at the Albany Med Comprehensive Stroke Center. This is a great opportunity to join our multidisciplinary team to make a profound impact on advancing care for stroke patients in the region.

Available position is in the Vascular Neurology/ Stroke Division of the Neurology Department, Albany Medical College, Albany NY. As an advanced practitioner, the NP will work in collaboration with the attending physician, primary nurse and other health care providers to provide stroke care. The NP is responsible and accountable for the development and application of specialty practice standards, and research to enhance the quality of care delivered to the patient. The NP will be responsible and accountable for the assessment and management of patients, including ordering diagnostic tests, consultations, therapeutic interventions and the day-to-day implementation and evaluation of an appropriate plan of care.

Essential Duties and Responsibilities

Available position provides an opportunity to be involved in both inpatient and outpatient stroke care, fast-track TIA clinic, participate in neuro-cardiac collaboration and other key program development initiatives of the vascular neurology division. Candidate will be working closely with the attending physicians and other APPs in the vascular division.

Inpatient care: Perform comprehensive neurologic assessments for patients with Acute ischemic stroke, TIAs, and Hemorrhagic Stroke. Order and interpret pertinent neurological findings. Collaborate with neurosurgery, ER, intensivists, therapists, and nursing staff. Implementation of acute stroke treatment plans and evidence-based protocols. Participate in daily inpatient rounds and clinical documentation of progress notes and discharge summaries.

Outpatient care: Provide post stroke and TIA follow up care. Monitor neurologic recovery and functional outcomes. Coordinate rehabilitation needs. . Provide long-term education and manage secondary stroke prevention. Medication adjustments. Coordinate referrals to other disciplines.

Administrative/Quality responsibilities: Maintain stroke center compliance with evidence-based standards. Participate in AMHS quality improvement initiatives. Assist with stroke metrics tracking and stroke center accreditation activities. Participate in monthly stroke conferences/case reviews.

Qualifications

• Master's Degree of an accredited Nurse Practitioner program.
• NP– Licensed Upon Hire.
• Prior experience is preferred but not required. Mentorship and supervision provided.
• Excellent interpersonal, verbal, and written skills.
• Successful completion and maintenance of ACLS and BLS.
• Able to quickly master all aspects of a computerized medical recordkeeping and billing system.
• Must project a professional image and earn the confidence of others.
• Excellent interpersonal, verbal, and written skills.
• Must be HIPAA compliant at all times.
• ATLS - Advanced Trauma Life Support Upon Hire - required
• BCLS - Basic Life Support Upon Hire - required
