Summary of Position:

We are seeking a highly motivated and clinically skilled Advanced Practice Provider (APP), either a Nurse Practitioner (NP) or Physician Assistant (PA), to join our Cardiology team. This full-time position provides a unique opportunity to practice across both inpatient and outpatient settings while collaborating closely with cardiologists and multidisciplinary healthcare professionals to deliver exceptional cardiovascular care.

The APP will participate in the evaluation, diagnosis, treatment, and ongoing management of patients with a wide range of cardiovascular conditions. Responsibilities include hospital rounding, outpatient clinic management, patient education, care coordination, and participation in a shared call schedule.

Essential Responsibilities:

Inpatient Responsibilities

  • Conduct comprehensive cardiovascular assessments and develop treatment plans in collaboration with supervising physicians.
  • Perform hospital rounds on cardiology patients, including admissions, consultations, and follow-up care.
  • Monitor patient progress and adjust treatment plans as appropriate.
  • Interpret diagnostic studies, laboratory results, and cardiac testing.
  • Coordinate care with consulting services, nursing staff, case management, and other healthcare providers.
  • Facilitate patient and family education regarding diagnoses, treatment plans, medications, and lifestyle modifications.
  • Assist with discharge planning and transitions of care.

Outpatient Responsibilities

  • Independently manage scheduled clinic patients within the APP scope of practice.
  • Evaluate and treat patients with acute and chronic cardiovascular conditions.
  • Provide follow-up care for patients with heart failure, coronary artery disease, arrhythmias, hypertension, valvular disease, and other cardiac disorders.
  • Order, interpret, and communicate diagnostic testing results.
  • Optimize medical therapies and monitor treatment outcomes.
  • Promote preventive cardiology initiatives and patient education.

Schedule and Call Responsibilities:

  • Combination of inpatient and outpatient clinical responsibilities.
  • Participation in shared night call rotation.
  • Weekend coverage approximately one (1) out of every eight (8) weekends.
  • Call schedule designed to support work-life balance while ensuring continuity of patient care.

Qualifications:

Required
