Who are you?You are a caring and dedicated professional that has completion of accredited PNP/PA program that includes clinical experiences in all aspects of primary healthcare across all age groups. At least one year of post degree experience is preferred but not required. Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program with current licensure as a Registered Nurse and Advanced Practice Registered Nurse (APRN) in the state of Ohio and national certification as a Family Nurse Practitioner (FNP) or current licensure as a Physician Assistant (PA) in the state of Ohio and current national certification as a Physician Assistant.
You will possess skill and experience in assessment and treatment of patients while working with multi-disciplinary teams. You will demonstrate the ability to work with diverse populations as well as cultural competence.
In this role, you will regularly use various computer apps including Microsoft Office, our HRIS, EHR, and other online systems, with multiple logins often simultaneously.
You have the ability to obtain a Driver's License, maintain auto insurance, earn CPR/First Aid certification, and be willing to submit to background check and fingerprinting.
How you'll make a difference in this role:You will practice within a Team Based Model, ensuring integrated care that addresses primary and behavioral health conditions through direct partnership with all disciplines involved in the treatment of clients. Your primary role is the provision of direct care services including review of health assessment forms, completion of comprehensive mental health evaluations, establishing diagnoses, determining appropriate treatment plans, provide ongoing care and making appropriate referrals. While in this role you will include the following:
- Provide comprehensive primary care services, including health assessments, physical examinations, diagnosis, and treatment of acute and chronic illnesses.
- Develop and implement individualized treatment plans based on patient needs and evidence-based practice.
- Order, perform, and interpret diagnostic and laboratory tests.
- Prescribe medications, treatments, and therapeutic interventions within the established scope of practice.
- Provide preventive healthcare services, including routine screenings, immunizations, and health risk assessments.
- Educate patients and families regarding disease prevention, health maintenance, medication management, and healthy lifestyle choices.
- Coordinate and follow up on referrals to specialists, diagnostic services, and community resources as appropriate.
