Full Job Description
JOB SUMMARY

  • To provide education and training to facility staff, as it relates to the best practices, current facility policy and procedures, and State and Federal regulations. Provide comprehensive clinical advice to facility staff in a manner that promotes positive resident and patient outcomes.


QUALIFICATIONS

  • Must possess a current, unencumbered License to practice as a Registered Nurse in the State of Illinois or Wisconsin.
  • Bachelor's Degree in nursing preferred.
  • Previous clinical and supervisory experience in Nursing.
  • Rehab Certified Nurse preferred.
  • Must possess current CPR certificate or become certified within Ninety (90) days of employment. Certified CPR Instructor preferred.
  • Must be able to read, write, and speak the English language in an understandable manner.
  • Must possess leadership, supervisor ability. Willingness to work harmoniously with all personnel.
  • Must possess the ability to make independent decisions and to take directive action when circumstances warrant such action.
  • Must possess a valid Illinois Driver's License, vehicle, and the willingness to travel within the City of Chicago and the surrounding suburbs and other areas as designated.
  • Possess excellent communication abilities though listening, speaking, and writing skills.


RESPONSIBILITIES

  • Accepts and supports the Board of Directors philosophy of care, including the mission, standards, and values.
  • Consult with facility's Director of Nursing in planning, organizing, implementing, evaluating, and directing the nursing department's activities in a manner that promotes positive clinical outcomes and alignment with the mission, standards, and values.
  • Attend and participate in relevant AMS Meetings.
  • Attend and participate in Continuing Education Programs.
  • Provide corporate-wide and facility-specific in-service education as needed in the area of nursing, record and file, infection control practice, Medicare; and coordinates educational needs with the Staff Training and Development partners.
  • Consult with facilities in meeting budgetary requirements for staffing (monitors and guides on serious overage and under clinical staffing), supplies, and consulting services for nursing and record and file departments.


JOB DUTIES

  • Visit each facility at least based on facility need.
  • Responsible for clinical components of any internal or external facility audits and follow-up Plan of Corrections.
  • Responsible for monitoring of clinical programs, including: Skin Management, Incident and Accident management, Weight Management, Pain management, and Psychotropic Programs.
  • Responsible for key metrics for Pressure Sore prevention, Incident/Accident management and Returns to the Hospital.
  • Monitor facility's clinical regulatory compliance.
  • Identify and coordinate timely educational opportunities for nursing staff that would strengthen clinical performance. (i.e. Ostomy training, Trach training, Wound Vac,, and other unusual medical conditions)
  • Submit monthly metrics report to VP of Operations and Director of Clinical Services.
  • Review all unusual occurrences and incidents of serious nature upon discovery and coordinates compliance with necessary follow up.
  • Responsible for covering facilities in the absence of DON.
  • Responsible for ensuring the completion of the orientation and training of the DON and ADON(s).
  • Responsible for oversight of documentation capture related to clinical components of Public Aid and Medicare Reimbursement Programs.
  • Assist other nurse consultants when needed.
  • Support all facilities when Illinois Department of Public Health, Wisconsin Department of Health Services or outside auditing agency is in the building.
  • Perform chart audits for all facilities on an on-going basis and follows up to ensure corrective actions have been implemented.
  • Assist with recruitment and retention when needed.
  • Conduct clinical rounds when visiting facility.
  • Review customer care/concern resolution information and assist with developing preventative processes to reduce and/or eliminate clinical concerns. Uses One Degree of Separation concepts to assist with this process.
  • Verify that AMS processes are being followed in clinical program areas including: Skin Management, Incident and Accident management, Weight Management, Pain management, Psychotropic Programs, and Returns to the Hospital.
  • Responsible for overseeing and assist with directing (when needed) QAPI processes related to clinical processes and outcomes.
  • Assist with developing and monitoring corrective plans for processes that are not compliant.
  • Coordinate clinical rounds on off hours (late p.m. or weekend) as needed based on identified facility concerns.
  • Observe nurses and CNAs in key competency areas on an on-going basis.
  • Must have working knowledge of all electronic software components to assist in monitoring clinical services (Point Click Care, Risk watch, Care Watch, Laserfische, Dashboards, Policy and Procedure manual, etc.)
  • Assume a leadership role during new acquisitions and/or new construction by organizing and implementing clinical systems.
  • Perform other related duties as assigned.
