Position Details:
Hartford HealthCare's Neighborhood Health program is seeking passionate per diem Advanced Practitioners to join our primary care team dedicated to improving health equity in underserved communities. This role offers the opportunity to provide high-quality preventative and urgent care through mobile and community-based services, addressing health disparities and promoting wellness. Role: Open to Family Nurse Practitioners and Physician Assistants
Location: Neighborhood Health covers various locations across CT. This position requires travel to our home base hubs out of Hartford and New Britain then travel in company fleet to clinic sites and Street Medicine.
Setting: 100% Outpatient Primary Care/Urgent Care - Family Medicine
Schedule: Full-time, weekdays only.
Call schedule: No call required

Nurse Practitioners and Physician Assistants Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider's practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physicians.

Licensure: Unrestricted State of Connecticut Medical License (or eligible)
Training: Family Nurse Practitioner (FNP) or Physician Assistant (PA)
Experience: 2+ years as a Family Nurse Practitioner or Physician Assistant, preferably in a primary care or urgent care setting.
• Valid driver's License within 3 Months (90 days) of hire date or job transfer date
• Ability to push/pull/lift 50 lbs
