Required Qualifications:
Advanced Practice Provider: Nurse Practitioner (NP) or Physician Assistant (PA)
Infectious Disease experience preferred, but motivated new graduates with strong clinical training are encouraged to apply
Strong clinical assessment and diagnostic skills
Passion for delivering patient-centered care in a collaborative team environment
Commitment to evidence-based medicine, infection prevention, and quality improvement
Position Highlights:
Provide inpatient and outpatient Infectious Disease follow-up care
Evaluate and manage patients with acute and chronic infectious conditions including sepsis, bacteremia, osteomyelitis, hepatitis, and complex infections
Collaborate with physicians, pharmacists, nursing staff, and care management teams to develop individualized treatment plans
Assist with antimicrobial stewardship initiatives and infection prevention strategies
Participate in patient education, transitions of care, and coordination with referring providers
Utilize Epic EHR and integrated clinical tools to support efficient, high-quality patient care
