Required Qualifications:- Nurse Practitioner or Physician Assistant certification and licensure
- Prior experience in endocrinology or chronic disease management preferred
- Strong interest in endocrinology and long-term patient care
- Commitment to high-quality, patient-centered care and collaborative practice
Position Highlights:- Evaluate, diagnose, and manage patients with endocrine disorders including diabetes, thyroid disease, adrenal and pituitary disorders, osteoporosis, and metabolic conditions
- Provide ongoing management and medication optimization for chronic endocrine conditions
- Educate patients and families on disease management, lifestyle modification, and treatment adherence
- Collaborate closely with endocrinologists, primary care providers, nursing staff, and care coordinators
- Support outpatient-focused endocrinology services with potential inpatient consultation support as needed
- Document care in Epic EHR and participate in quality improvement and care optimization initiatives
