Description of Work
This position coordinates the Minimum Data Set (MDS) system according to CMS certification and licensure standards on all residents. Coordinates and serves as Team Leader for the Treatment Team on the assigned unit. Serves as teaching role as needed to maintain skills for clinical disciplines on MDS and care planning. This position is considered mandatory, which means employees are required to report to work during adverse weather and emergency conditions.

Recruitment Range: $62,044 to $102,371

  • Possess leadership ability.


*A Sign-on bonus of $10,000 may be offered to the successful applicant(s) of this posting, contingent on the [redacted] Budget Approval.

*Eligibility for Sign-on Bonus

To qualify for the Sign-on Bonus, a newly hired permanent full-time or permanent part-time employee must work 30 to 40 hours per week to be eligible. Those who are eligible and working less than 40 hours a week will receive a prorated rate.

An employee is not eligible for a sign-on bonus if:
• The employee has held a position in the North Carolina State government, including the judicial system, or the University of North Carolina system, within the last 12 months (the employee shall remain ineligible for 12 months from the separation date).
• The employee has previously received a sign-on bonus from any State agency.
• DHHS has paid any contract placement or recruitment fees.

Longleaf Neuro-Medical Treatment Center (LNMTC), located in Wilson, NC, is a state-operated certified Skilled Nursing Facility (SNF). With a staff of about 520, LNMTC provides support to approximately 200 residents with complex medical needs and/or co-existing neuro-cognitive disorders.

THIS POSITION IS DESIGNATED MANDATORY AND MUST REPORT OR STAY OVER DURING ADVERSE WEATHEREVENTS OR OTHER EMERGENCIES.
