RN MDS Coordinator to support assessment, care planning, and clinical operations in a skilled nursing setting. This is a
full-time role with a balanced mix of MDS coordination and hands-on clinical support.
Schedule Structure- Approx. 24 hrs/week: MDS Coordination
- Approx. 16 hrs/week: Resident Care Manager (RCM) support
- Additional floor coverage as needed (call-ins, staffing support)
Key Responsibilities- Coordinate MDS, CAAs, and care plans
- Set ARDs and manage MDS scheduling
- Ensure accurate documentation and regulatory compliance
- Collaborate with IDT and support Medicare processes
- Assist with unit oversight and clinical support
Qualifications- Active RN license (OR or ability to obtain)
- Skilled nursing/LTC experience preferred
- MDS/RAI knowledge strongly preferred
- EMR experience (PCC a plus)
