Licensed Practical Nurses (LPNs) - Home CareOrganization: Affirmed Home Care - Delivering Excellence in Home Health Services
Location: New York City & Long Island (Manhattan, Bronx, Brooklyn, Queens, Nassau & Suffolk Counties)
Setting: In-Home Pediatric & Adult Care
Key Responsibilities- Provide skilled nursing care, including medication administration, wound care, catheter care, and IV therapy
- Conduct patient assessments and contribute to individualized care planning
- Monitor patient conditions and report changes appropriately
- Educate patients and family members regarding treatment plans and health management
- Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards
- Ensure patient safety while upholding the highest standards of nursing practice
- Collaborate with interdisciplinary healthcare teams and community providers
Qualifications- Active New York State LPN License
- Minimum of 1-2 years of nursing experience preferred (Home Care, Pediatrics, or Geriatrics)
- Strong clinical judgment and ability to work independently
- Current CPR/BLS Certification
- Experience with Electronic Health Records (EHR) preferred
- Excellent communication, organizational, and interpersonal skills
- Reliable transportation and/or valid driver's license preferred
