Full Job Description
We are hiring Licensed Practical Nurses (LPN) to join our dynamic care team at Plymouth Rehabilitation & Health Care Center in Plymouth! As an LPN, you will play a vital role in providing high-quality nursing care to our residents. We offer a competitive hourly rate of $40 to $46 with weekly pay, shift differentials, and an up to $5,000 sign-on bonus!*

Full-time and part-time openings on all shifts.

Responsibilities:
  • Provide direct nursing care under the supervision of the RN to residents and supervise the day-to-day nursing activities performed by the nursing assistant
  • Meet the nursing and rehabilitation needs of each resident
  • Maintain current resident care plans on all residents under your care
  • Follow physician's orders and administer medication according to procedure
  • Document comprehensive and complete nursing notes
  • Visit each resident daily to evaluate resident needs
  • Maintain accurate awareness of residents on alert list
  • Document and report any unusual or significant findings
  • Collaborate with the healthcare team to provide exceptional care

Requirements:
  • Must be a graduate of an approved LPN/LVN program
  • Must be CPR certified
  • Must possess a current, unencumbered license to practice as an LPN/LVN in this state, or be in the process of obtaining one
