Responsibilities
Promote knowledge of oncology for patients and support partners
Meet with patients post-diagnosis to provide counseling and referrals
Collaborate with a multidisciplinary team to support patients and oversee care plans
Educate the team on quality initiatives and research projects
Assess patients for emotional needs and risk factors
Act as a customer service advocate for patients and support partners
Serve as the primary contact for providers and patients needing OLSW services
Work with Navigators to create Survivorship Care Plans

Job Description:
Promotes knowledge of oncology and its effects for patients, support partners, and care providers.
Meets with patients and support partners after diagnosis to provide counseling and referral services specific to their needs.
Collaborates with the multidisciplinary team to provide ongoing support for patients and oversee the plan of care.
Educates and collaborates with the multidisciplinary team to plan and participate in quality initiatives and research projects.
Reviews patient information and assess patients for emotional needs and risk factors.
Serves as a customer service advocate for patients and support partners.
Serves as the primary point of contact for providers and patients requiring the Oncology Licensed Social Worker (OLSW).
Works with Navigators to provide Survivorship Care Plans to patients.
Performs patient and family evaluations and histories, providing psychosocial support through individual or family counseling as needed.
Acts as a resource to Infusion/Oncology Practices for patient needs and collaborates with patients to identify potential barriers to achieving oncology treatments and set health goals for quality of life.
Other duties as assigned.
