Responsibilities

  • Provide pharmaceutical care and medication management to patients
  • Collaborate with healthcare team members to optimize medication therapy
  • Educate patients on proper medication use and potential side effects
  • Ensure compliance with state and federal pharmacy regulations
  • Manage pharmacy operations and oversee pharmacy staff
  • Participate in continuous improvement initiatives related to pharmacy services
  • Complete required annual education related to specialty accreditation standards, if applicable

MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:

Must possess current license as required by state board where services will be provided:
WV: Pharmacist license through the West Virginia State Board of Pharmacy
PA: Pharmacist license through the Pennsylvania State Board of Pharmacy
MD: Pharmacist license through the Maryland Board of Pharmacy
OH: Pharmacist license through the Ohio Board of Pharmacy
Remote Workers Only: Must additionally hold current Pharmacist license issued by state of residence.

EXPERIENCE:
1. Five (5) years' experience as pharmacist in a pharmacy setting.

PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Doctor of Pharmacy (Pharm.D.) from an ACPE accredited School of Pharmacy.
2. Board Certification.

EXPERIENCE:
Three (3) years' experience in leadership or management role.

For facilities with specialty accreditation requirements:

1. Completion of annual required education related to specialty accreditation as defined by accreditation standards. Training may be completed through CBLs, trainings, In-services, and competency validation.
