About the position: Stellar's Nova business bears total cost of care risk on behalf of a growing portfolio of ACO and Payor risk contracts. The Nova Performance function owns the aggregate results of that risk - quality, total cost of care, and shared savings - and partners with Provider Success to translate network-wide performance goals into action at the provider level.
The Lead, ACO Performance is the general manager and performance owner for two of Stellar's most important Medicare ACO relationships: our new participation in CMS's LEAD (Long-term Enhanced ACO Design) Model, the successor to ACO REACH, and our partnership with the affiliated provider network of an academic health system.
How you'll make an impact: - Own ACO Performance End to End: Serve as the general manager of two CMS ACO accounts, accountable for aggregate quality performance, total cost of care, and shared savings/risk results across both programs.
- Set Targets & Define Success: Define benchmarks and "definition of success" for each ACO in partnership with Analytics, Med Econ and functional leadership - translating what's feasible into concrete, trackable operational targets.
- Lead Governance & Executive Reporting: Own the agenda, materials, and facilitation for steering committee and operating reviews with each ACO's leadership, and build the executive reporting cadence used to track progress internally.
- Drive Performance Analysis: Analyze quality, claims, and total cost of care data to identify drivers of under- and over-performance, and translate findings into clear, actionable recommendations with an explicit "so what."
- Partner on Provider-Level Execution: Work closely with Provider Success counterparts assigned to each ACO's network to ensure every identified performance gap has a data-driven action plan, and that plans are executed and tracked to completion.
- Own the SVU & Incentive Strategy: Recommend and refine the SVU menu and incentive design for each ACO to optimize outcomes for Stellar, the ACO, and its providers.
- Navigate Program & Model Requirements: Stay current on LEAD Model program requirements and milestones (e.g., benchmarking, risk track elections, reporting obligations) and coordinate internally to ensure Stellar and its ACO partners stay ahead of them.
- Escalate & Problem-Solve: Proactively identify the greatest risks to each ACO's performance, and escalate to the VP, Nova Performance with a clear, well-reasoned plan rather than letting underperformance linger unaddressed.
- Contribute to the Broader Function: Share best practices and lessons learned from both ACOs across the Nova Performance team, and help build the playbooks and tooling that let performance management scale as Stellar's ACO portfolio grows.
What you'll bring:- 4+ years of experience in healthcare operations, value-based care, ACO/managed care, or account management within a health-tech or provider services environment.
- Knowledge of Medicare total cost of care risk models - MSSP, ACO REACH, the new LEAD Model, or similar - strongly preferred.
- A track record of owning a book of business or a set of accounts end to end, operating largely autonomously and delivering measurable outcomes with limited day-to-day oversight.
- Comfort leading executive-facing meetings (steering committees, operating reviews) and communicating performance and risk clearly to both internal leadership and external partners.
- Strong analytical skills, including fluency in Excel and BI tools, and the ability to turn performance data into clear insight and a concrete "so what."
- The ability to work independently against ambiguous problems, set your own short-term priorities, and proactively flag when you need input on longer-term direction.
- Excellent written and verbal communication skills, with the ability to build trust quickly across internal cross-functional teams and external stakeholders, from operational staff to health system executives.
- A collaborative mindset - you're comfortable driving outcomes through influence and partnership with other functions, rather than direct authority alone.
