As a Clinical Services Manager, you will oversee care coordination, guide clinical staff, and ensure exceptional service delivery across home health and hospice programs.
What You'll DoAs a Clinical Services Manager, you will provide leadership and oversight to ensure quality care and efficient operations. Responsibilities include:
- Managing patient referrals, reviewing cases, and assigning appropriate clinical staff
- Overseeing the development, implementation, and updates of individualized plans of care
- Collaborating with physicians and the Medical Director regarding patient eligibility and care needs
- Supporting clinicians with goal setting, care planning, and clinical decision-making
- Triaging and addressing concerns from patients, families, facilities, and providers
- Approving and managing clinical workflows for home health and hospice services
- Conducting home visits, record reviews, and performance evaluations to ensure quality care
- Leading and participating in interdisciplinary team (IDG) meetings and case conferences
- Performing chart audits, supervisory visits, and quality improvement initiatives (QAPI)
- Supporting hiring, onboarding, and development of clinical staff
- Reviewing DME and pharmacy orders for accuracy and appropriateness
- Monitoring outcomes and educating staff to support performance improvement
- Managing compliance processes such as Face-to-Face (F2F) visits
- Participating in leadership on-call rotation and completing visits as needed
What We're Looking For- Current Registered Nurse (RN) license in the State of Wisconsin
- BSN preferred
- Home health or hospice experience preferred
- Minimum of 2 years of leadership or supervisory experience preferred
- Strong clinical judgment, communication, and organizational skills
- Ability to lead and collaborate within an interdisciplinary team
- Understanding of hospice philosophy and end-of-life care
- Knowledge of pain and symptom management
- Ability to manage multiple priorities in a fast-paced environment
- Valid driver's license with a reliable, insured vehicle
