Schedule: Full-TimeMonday-Friday, 8:00 AM - 5:00 PM1 Weekend Per Month1 On-Call Shift Per Week
Compensation: $85,000 - $130,000 annually plus

What You'll Do- Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
- Complete OASIS assessments and individualized plans of care.
- Provide skilled nursing visits, assessments, and patient education.
- Coordinate care with physicians and the interdisciplinary team.
- Help patients remain safe and independent in their homes.
- Maintain accurate and timely clinical documentation.
What We're Looking For- Active Minnesota Registered Nurse (RN) license.
- 3+ years of RN experience (home health experience preferred).
- Strong clinical judgment and ability to work independently.
- Excellent organization and communication skills.
- Reliable transportation for travel throughout the territory.
