Position: Full-time Home Health Occupational Therapist (OT)

Responsibilities- Provide comprehensive patient care through functional assessments, identification of performance deficits, and development of client-centered treatment plans in home health settings.
- Develop and implement individualized OT care plans focused on improving ADLs, IADLs, safety, participation, and environmental adaptations.
- Educate patients and families on therapeutic techniques, compensatory strategies, energy conservation, home exercise/engagement programs, and safe use of adaptive equipment.
- Collaborate with interdisciplinary teams to ensure continuity of care, effective communication, and safe discharge planning.
- Maintain accurate documentation of patient progress, functional outcomes, and clinical reasoning while adhering to professional OT standards and medical terminology.
- Stay current with best practices in occupational therapy, particularly in home health, geriatrics, functional rehabilitation, and aging-in-place strategies.
Qualifications:- Licensed Occupational Therapist in Washington state (or before your first day of work)
- One to two years of experience as a Occupational Therapist
- Current BLS certification
