$125k-$140kWhat you'll do:- Lead and manage all clinical operations
- Supervise, mentor, and develop clinical managers and field staff, fostering a culture of collaboration and excellence
- Ensure full compliance with Medicare, Medi-Cal, and state home health regulations, including Conditions of Participation
- Oversee OASIS accuracy, care planning, and documentation to support quality outcomes and appropriate reimbursement
- Partner with executive leadership on strategic growth, performance metrics, and process improvements
- Serve as a clinical resource and trusted point of escalation for patients, families, and referral sources
What you bring:- Must have: Active RN license in California (BSN required; MSN or equivalent preferred)
- Must have: 5+ years of progressive nursing leadership experience in home health, including oversight of multi-disciplinary teams
- Must have: Deep knowledge of Medicare home health regulations, OASIS, and PDGM reimbursement
- Must have: Proven track record in survey readiness, quality assurance, and performance improvement
- Must have: Exceptional communication and relationship-building skills with staff, physicians, and referral partners
- Nice to have: Certification in Home Health Nursing (e.g., COS-C, CHPN) or Nursing Administration
- Nice to have: Experience with EMR systems used in home health (e.g., Homecare Homebase, PointClickCare)
