Qualifications

• Licensed Physical Therapist (PT) in Hawaii
• 1+ years of experience in clinical or home-based care
• Preferred experience with fall risk assessments
• Excellent communication and interpersonal skills
• Valid driver's license and reliable transportation

Responsibilities

• Deliver personalized physical therapy services to home-based clients
• Conduct initial and ongoing assessments to track progress and adjust treatment plans
• Perform fall risk assessments at Assisted Living Facilities (ALFs) and community expos
• Supervise client care to ensure best practices and quality outcomes
• Provide educational presentations at ALF events and expos
• Travel to client locations and events as needed
• Create and update care plans for non-medical clients
• Handle administrative duties including calling PCP offices for orders
