POSITION DETAILS- Position: Licensed Practical Nurse (LPN)
- Employment Type: Full-Time
- Location: Patchogue, Suffolk County, NY
- Setting: Facility Floor Staffing
- Pay Rate: $40-$45/hour, based on experience and assignment
- Availability: Full-time schedule based on facility staffing needs
RESPONSIBILITIES- Provide quality nursing care and support to residents.
- Administer medications and treatments within the scope of the LPN license.
- Monitor residents' conditions and report changes to the appropriate healthcare professionals.
- Maintain accurate and timely documentation using the facility's charting system.
- Follow facility policies, procedures, and infection control protocols.
- Collaborate with other healthcare professionals to promote resident safety and well-being.
- Assist with other nursing duties as assigned within the scope of practice.
QUALIFICATIONS- Valid New York State LPN license required.
- Previous nursing experience preferred.
- Long-term care or facility experience preferred.
- Reliable, professional, and patient-centered.
- Strong communication and documentation skills.
- Must be authorized to work in the United States.
- Must have updated medical and vaccination records or be willing to obtain them.
- Must complete the required drug screening or habituation form.
- Must successfully complete Alvita Care's onboarding process before starting the assignment.
- Must be able to meet the facility's scheduling and staffing requirements.
