Full Job Description
The Carefirst Family Health Team (FHT) is seeking full-time/part-time permanent and locum family physicians to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.

On-site & Integrated Programs/Services include:
  • Ontario Health atHome (formerly Home and Community Care Support Services)
  • MINT Memory Clinic
  • Ontario Structured Psychotherapy (OSP) Program
  • Specialist Clinics: psychiatry, cardiology, endocrinology, optometry, geriatric medicine, gynecology, chiropody, rheumatology, physiotherapy, and audiology

Clinic Locations

We currently operate four well-established clinics across Scarborough, Markham and Richmond Hill, and continue to expand in Markham and Stouffville to support continued growth and community needs.

Physicians will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.

The Position

With this rapid expansion, we are looking for compassionate, team-oriented family physicians who are committed to delivering high-quality care and value interdisciplinary collaboration to fill a new position at our Family Health Team.

An existing patient waitlist offers an excellent opportunity to establish a thriving practice quickly. We welcome new graduates and international medical graduates fulfilling return-of-service requirements!

  • Innovative and well-established system of team-based, community-focused family medicine practice with 12,000+ rostered patients across sites
  • Flexible patient panel and practice design: Build a panel that aligns with a desired FTE, scope and professional goals. Panel Range: ~1,400 - 1,650+ patients (based on FTE)
  • Turnkey, no-overhead practice model: Full administrative, billing and multidisciplinary team support at no direct cost o the physician
  • Work-life balance: Physicians can maintain a sustainable clinical schedule with flexible FTE options, structured appointment times, and only one evening or weekend after-hour shift per week in compliance with Ministry requirement.
  • Digitally enabled primary care: Fully EMR-enabled practice (Telus Health), AI-powered medical scribe, online appointment booking, eReferrals, digital communication and growing, with on-demand training and support.
  • Efficient appointment structure: Standard appointments can be set up as 15-minutre visits and 30-minute periodic health review.
  • Access to broader services at Carefirst and wide network of health system partners, including on-site specialists, community supports, chronic disease prevention and management, and more.
  • Collaborative interprofessional team-based care: Physicians work alongside a full interprofessional team for comprehensive care for patients

Qualifications
  • Licensed, or eligible for licensure, to practice family medicine in Ontario and in good standing with the Collage of Physicians and Surgeons of Ontario (CPSO).
  • Certification in Family Medicine with the College of Family Physicians of Canada (CCFP), or equivalent eligibility, is preferred/required depending on practice arrangement.
  • Active OHIP billing number
  • Strong communication, clinical judgement, professionalism, time management and collaborative practice skills
  • Compassionate, approachable and comfortable working with diverse patients, families and interprofessional team members.
  • Interest in digital health, quality improvement, teaching, population health, preventive care, or integrated care is an asset.
  • Ability to provide care in additional languages, such as Cantonese, Mandarin and Tamil, is an asset.
