Family Medicine Physician Lincoln CA Sacramento CA

Job Description:

We are actively seeking a dedicated and compassionate Family Medicine Physician to join our growing clinical team. In this role, you will work shoulder-to-shoulder with our experienced staff and specialist network to provide comprehensive, multidisciplinary care all from one location. Whether managing a chronic illness, addressing an urgent medical problem, or performing wellness check-ups, you will be the cornerstone of our patients' health journeys.

Key Responsibilities:

  • Comprehensive Primary Care: Provide preventive care including annual physicals, well-child visits, immunizations, and sick visits for both children (ages 6 and older) and adults.
  • Acute Care Services: Offer urgent care, pediatric care, short-term stabilization, and post-hospital follow-up care designed to keep patients safe, independent, and out of the hospital.
  • Chronic Disease Management: Manage acute and chronic conditions, including providing medication therapy management, diet and nutrition counseling, and mobility/home medical equipment assessments.
  • Geriatric & Palliative Care: Provide specialized adult primary care and patient observation tailored to our senior citizen population.
  • Collaborative Practice: Work collaboratively with our clinical team and network of specialists to ensure comprehensive continuity of care.
  • Work Life Balance: Predictable work schedule, flexible full or part-time option, no after hours practice call.

Qualifications:
