Overview

Reporting to the VP of Nursing, the Division Director, Post-Acute Care will ensure patients served by UF Health Jacksonville can smoothly progress through their care needs from inpatient to post-acute in a safe and high-quality environment that is matched to their needs. The Division Director, Post-Acute Care maintains responsibility to govern and set priorities that ensure an exceptional, safe clinical care experience grounding in continuous process improvements across UF Health Jacksonville. This position will work to drive value as defined by the highest level of patient safety, quality, and experience of care while focusing on ensuring patients are cared for in the best setting for their needs in a cost-effective manner. In addition to oversight and facilitation of care in the acute setting through case management services, the Division Director, Post-Acute Care will oversee and effectively manage all business units within Post-Acute Care Services, the internal and external relationships with Post-Acute facilities, and other key strategic relationships that impact the continuum of care. This role serves as the hospital's primary contact for post-acute partners and is responsible for monitoring their performance and ensuring that desired outcomes are achieved.

Necessary Skills:
1. Serves as the Division Director, Post-Acute Care with oversight of the Transitional Care Unit (TCU), Case Management, Palliative Care, Utilization Management, Home Health, Hospital at Home, and the Transition of Care team. 2. Integrates the mission, vision, and values of UF Health Jacksonville across all Post-Acute Care Services. 3. Provides strategic leadership and direction for Post-Acute Care Services and facilities. Ensures the availability of necessary resources to deliver high-quality patient care. Fosters strong, collaborative clinical and operational partnerships between UF Health Jacksonville and community partners that interface or impact patient care. 4. Develops strategic internal and external relationships to enhance access, capacity, quality, service, and affordability across the continuum of care. 5. Recruits, motivates, develops, and retains a diverse, high-performing workforce. Works collaboratively to establish criteria for recruitment, selection, retention, performance assessment, promotion, and competence of all direct reports and staff. 6. Shares accountability for quality, safety, integration, and efficiency of clinical services and performances within Post-Acute Care Services. 7. Supports continuous improvement in patient care within a cost-effective environment, in collaboration with other leaders. Identifies quality improvement initiatives and oversees their implementation to enhance patient care services. Fosters a multidisciplinary approach to quality improvement, focusing on improved coordination of services and enhanced clinical outcomes. Ensures the patient care delivery system considers acuity, patient mix, community needs, staff knowledge and skills, available resources, and established healthcare benchmarks. 8. Defines, develops, and directs the implementation of goals and programs for patient and support services, including appropriate metrics. Actively participates in resolving patient concerns and complaints to achieve service excellence at the highest level. 9. Fully supports the development of all programs that promote patient safety and ensure clinical quality within all services and facilities. 10. Develops and manages budget performance, executing appropriate strategies to manage costs and improve the quality of care. 11. Assists with payor relationships and oversees utilization management initiatives and strategies. 12. Ensures compliance with all regulatory and accrediting bodies to meet requirements that impact all service facilities within the Post-Acute Care Services.
Required Licensure/Certifications:
License to practice Nursing in Florida Nursing Home Administration license in Florida
Required Experience:
A minimum of 7+ years of clinical care experience in a complex, academic medical center required. 5-7 years of progressive healthcare leadership responsibility required. Expert knowledge of post-acute methods, processes and practices relating to patient care and clinical outcomes.
Preferred Experience:
Experience in budget management, change management and process improvement is highly desirable.
