Bachelor's or Master's degree in Social Work preferred
Social Work License required
5-7 years of experience in Sub Acute and Long-Term Care
Familiarity with long-term care regulations and documentation
Experience in Skilled Nursing Facility or Long-Term Care setting preferred
Responsibilities
Complete MDS assessments
Facilitate care plan meetings and discharge planning
Implement History and Assessment, Advanced Directives, and End of Life Planning
Audit psychology paperwork, hospital discharges, and room changes
Complete all required documentation in the medical record (EMR)
Coordinate care and services with ongoing monitoring of person-centered care plans
Demonstrate judgment regarding safety and confidentiality issues

Responsibilities include, but are not limited to, the following:
Completing MDS assessments.
Facilitate care plan meetings & discharge planning.

Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills
