Responsibilities:
Oversee the development and implementation of social services programs and services, including but not limited to: case management, discharge planning, and resident assessment and care planning
Lead and manage a team of social workers, therapists, and other healthcare professionals to deliver high-quality care and services
Develop and maintain relationships with hospitals, physicians, and other healthcare providers to ensure seamless transitions and referrals
Coordinate and provide education and training to staff on social services best practices, regulations, and standards
Collaborate with other departments, including nursing, therapy, and activities, to ensure a comprehensive approach to resident care
Develop and manage budgets and staff schedules to ensure efficient and effective operations
Monitor and report on quality indicators, outcomes, and other performance metrics to inform decision-making and drive continuous improvement
