Full Job Description
Key Responsibilities

  • Direct and oversee the Social Services department for residents receiving short-term rehabilitation, long-term care, and skilled nursing services.
  • Provide consultation to Administration regarding program planning, policy development, and priority setting for social services in accordance with Massachusetts DPH, OBRA, and facility requirements.
  • Complete psychosocial assessments and develop individualized care plans that support each resident's emotional, social, and discharge planning needs.
  • Coordinate safe and effective discharge planning and connect residents and families with appropriate community resources.
  • Advocate for resident rights, dignity, autonomy, and quality of life.
  • Collaborate with the interdisciplinary team to ensure comprehensive, resident-centered care.
  • Maintain accurate documentation and ensure ongoing regulatory compliance and survey readiness.
