Job Description:

Schedule: Full Time, Salaried
Shift: Monday-Friday with Rotation Supervisor On-Call.
Location: Covering Central Florida Hospice. Office location in Altamonte Springs.

Communicates with other healthcare providers regarding admissions, including doctors, hospitals, assisted living facilities, and skilled nursing facilities. Reviews nursing documentation and charting to ensure it is timely, accurate, and complete, and communicates with the Health Information Management department to update incomplete documentation. Responds to significant circumstances escalated by managers or nurses to provide guidance and leadership and interpret policy. Works as a liaison between the Admissions/Referrals Departments and the Marketing Department, partnering with the Director - Business Development to streamline the process between marketing services and admitting hospice-qualified patients. Recommends and implements the fiscal budget of the assigned department, monitors budget, investigates variances, and reports findings. Participates in staff meetings, department meetings, in-services, educational and developmental seminars, and strategic plan subcommittees. Performs other duties as assigned. Provides overall direction and management of the Admissions and Referrals departments. Directs the development, implementation, and monitoring of department protocols, policies, and standards to ensure effective use of resources while maintaining patient care standards. Oversees the planning and organization of activities to ensure patient needs are met according to professional standards of medical and nursing care. Maintains knowledge and communicates policies and procedures required to comply with federal, state, and local guidelines, as well as Medicare, Medicaid, and insurance reimbursement requirements. Develops, reviews, and revises standards of care and creates new standards. Develops, revises, and implements schedules for admission nurses to ensure availability and coordinates staffing levels with Senior Referrals Manager or Teams.

  • Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
  • Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]
  • Knowledge of documenting within an electronic medical record. [Required]
  • Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software. [Required]
  • Knowledge of third party reimbursement and cost containment. [Required]
  • Knowledge of basic supervisory practices and procedures. [Required]
  • Ability to conduct interviews of prospective employees. [Required]
  • Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]
  • Ability to complete AdventHealth Hospice Care CFL Orientation and Training, as necessary. [Required]
