Responsibilities

• Collaborate with the sales team to identify and secure new patient referral sources.
• Maintain and implement coordinated care plans for patient transitions to alternative care sites.
• Act as a liaison between referring physicians and hospital case management teams.
• Conduct patient education visits prior to hospital discharge to discuss service eligibility.
• Document patient demographics and clinical information for accurate care management.
• Assist branch staff with necessary paperwork for therapy commencement and payer reimbursement.
• Complete administrative tasks like expense and referral tracking, and sales plans.
