Clinical Services Manager, RN - Home Health.

Position SummaryThe Clinical Services Manager is responsible for the day-to-day management and supervision of clinical operations and multidisciplinary home health teams. This position ensures timely, high-quality patient care, supports clinician development, promotes regulatory compliance, and assists in driving excellent patient outcomes.
Essential Responsibilities- Provide daily supervision and support to Registered Nurses, Licensed Practical Nurses, Home Health Aides, Physical, Occupational and Speech Therapists, and Medical Social Workers.
- Coordinate and monitor clinical operations and patient care delivery.
- Conduct case conferences and assist with care planning and coordination.
- Review clinical documentation, OASIS assessments, and physician orders for accuracy and compliance.
- Monitor patient outcomes, clinician productivity, and quality indicators.
- Provide education, coaching, and mentoring to clinical staff.
- Assist with quality assurance activities and survey readiness initiatives.
- Serve as a clinical resource to staff, patients, physicians, and referral sources.
- Promote teamwork, accountability, and exceptional customer service.
- Assist in developing and implementing clinical policies and procedures.
Qualifications- Current Illinois Registered Nurse (RN) license in good standing.
- Minimum of two years of supervisory or management experience in a Medicare-certified Home Health Agency.
- Strong knowledge of Medicare Conditions of Participation and home health regulations.
- OASIS knowledge and documentation experience.
- Excellent clinical assessment and critical-thinking skills.
- Strong leadership, communication, and organizational abilities.
- Excellent computer and electronic medical record proficiency.
- Ability to manage multiple priorities in a fast-paced environment.
