Are you looking for an opportunity to help shape a growing ambulatory pharmacy practice while improving outcomes for patients with chronic specialty disease states? Barnes-Jewish Hospital is seeking a Clinical Pharmacist Specialist to support a newly developed outpatient clinical service, collaborating with an interdisciplinary care team to optimize medication therapy, provide comprehensive disease state management, and improve access to specialty medications.

This role offers the opportunity to deliver direct patient care, support program development and quality initiatives, serve as a clinical resource for providers and patients, and contribute to the growth of ambulatory pharmacy services within a world-renowned academic medical center.

PGY2 residency preferred.

Here, department staff has access to a number of drug information resources, including many online references accessible from department computers. An on-site drug information center provides support for more in-depth questions.

Preferred Qualifications

Role Purpose

Evaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes, by working directly with physicians, other health professionals, and patients. Participates in the development, provision and evaluation of comprehensive pharmaceutical care and quality assurance programs through scholarly activities. Serves as a resource to the pharmacy staff and other hospital departments.

Responsibilities
  • Assess the overall status of the patient's health condition and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care. Documents all activities and communication as appropriate.
  • Participates in professional development and research activities.
  • Contributes to performance improvement, clinical effectiveness, drug use evaluation, and HSO/BJC committee activities.
  • Participates in the education and training of pharmacy staff, residents and students, and serves as a role model and mentor.
  • Participates in the identification and implementation of targeted drug cost reduction programs.
