Clinical Pharmacist

Coal Country Community Health Center (CCCHC) & Sakakawea Medical Center (SMC). Full-time, primarily day shifts.

To best serve our community and the patients we care for, we are seeking a Clinical Pharmacist who is committed to excellence, growth, and patient-centered healthcare. The Clinical Pharmacist provides comprehensive pharmaceutical care to patients served by CCCHC and SMC. This role supports both outpatient and inpatient pharmacy operations, ensuring safe, effective, and evidence-based medication use. The Pharmacist works collaboratively with providers, nurses, and ancillary staff to optimize patient outcomes in a rural, team-based care environment.

This position is primarily focused on ambulatory pharmacy practice and offers a unique opportunity to help expand and enhance clinical pharmacy services across the outpatient setting. We are seeking a clinically driven pharmacist who is passionate about improving patient outcomes through direct patient care, provider collaboration, chronic disease management, medication optimization, and innovative ambulatory care initiatives. While the role is largely ambulatory in nature, the ideal candidate will also possess knowledge of inpatient pharmacy operations and be able to collaborate effectively across the continuum of care to support both community health center and hospital-based services.
